[ts-gen] changes from version shim-091030 to newer versions

I tried to run the newest trading-shim version but it failed with versions newer then 091030.
make compiled all versions with success.

Trying to start shim:

shim-091030 $ ./shim

Usage: shim <mode> [optional feature list]
Modes:

               # real modes, requiring access to an IB tws:
    --data     # process subscriptions and log resulting tick stream events
    --risk     # accept full command set, send requests, and log all events



shim-091102 $ ./shim
terminate called after throwing an instance of 'MinimalModeMultiParadigm::allocation_failure'
Abgebrochen



shim-091123 $ ./shim
terminate called after throwing an instance of 'MinimalModeMultiParadigm::block_must_be_page'
Abgebrochen
